The Advanced Elderly Medical Service System

The Advanced Elderly Medical Service System is a system for people 75 years old or older and for people who are 65 years old or older and have a certain disability. One medical insurance certificate for the Advanced Elderly Medical Service System is issued per person (sent before the person’s 75th birthday). It states the percentage you bear so please make sure to present it when going to a medical facility.

You cannot enroll in the Advanced Elderly Medical Service System if:

  • You are not registered as a resident of Toshima City
  • Your period of stay is 3 months or less
  • Your visa status is for those engaged in “designated activities” and you are involved in “activities with medical treatment” or “activities taking care of the person daily who receives medical treatment,” or you are engaged in “activities for sightseeing, recreation or similar”
  • You receive welfare benefit

Notification for the Advanced Elderly Medical Service System

Please notify us within 14 days if any of the following occurs:

  • You moved into Toshima City
  • You are moving out of Toshima City
  • Your address has changed within Toshima City or your name has changed
  • The enrollee dies
  • You have started receiving welfare benefit, or, you have stopped receiving it
  • You lost your medical insurance certificate ※In this case, please notify us immediately

Premiums

Each enrollee pays a premium. As a general rule, the premium is directly taken out of the retirement benefits. You will need to follow procedures to enroll in bank transfer.

Payment consultation

If you are unable to make payments by the due date because of certain circumstances, please come consult with us instead of leaving it unpaid.

Benefits

Cost at the reception desk of the medical facility is 10% or 30% (for an income earner of a certain amount). Please present your health insurance card at the reception desk of the medical facility when receiving care. When you do not present it, you will have to pay for the entire treatment for the time being.

Other benefits

There are repayments for when you pay high medical costs or care taking insurance fees and disbursements of funeral expenses for when the enrollee in the system passes away.
