My Number Notification Cards and My Number Cards

  What is an Individual Number?

The Individual Number System (commonly known as “My Number”) is a unique 12-digit number that is assigned to each person who possesses a Resident Record. In order to prevent specific persons being identified by their number, numbers are assigned randomly with no relation to identifiable factors such as home address or date of birth, etc. The My Number system offers a number of advantages. It helps realize a fairer and more just society as well as enhances the convenience of the nation’s citizens. It also helps improve administrative efficiency. 

About Notification Cards

Once you have completed the required resident registration procedures, if you are newly-arrived in Japan, your Notification Card with your My Number will be sent to you, using simple registered post and no forwarding service to the address indicated on your Resident Record. Your My Number is very important and necessary for living in Japan. To make sure that you receive it, you are encouraged to confirm the following points:

  • Please make sure to display your name on your letterbox or on the front door of your home. Furthermore, please file a “notice of change of address” with the post office in order that you may continue to receive your mail without disruption.
  • As for simple registered post, you can receive it by signing for it when the post office delivers it to your home. If you are absent during the day, it is requested that upon returning home, that you check your letterbox to see if the post office has left a “delivery notice for items of mail, etc.” If such a notice has been left, please contact the post office to arrange for them to redeliver your Notification Card.

If it is not delivered or if the post office storage period indicated on the “delivery notice for items of mail, etc.,” expires, a Notification Card will be returned to the Toshima City Office. If that happens in your case, please come and pick up your Notification Card to the General Information Center Section at the Toshima City Office with your Residence Card.

 What to do when you receive your Notification Card

Please confirm the following three items are in the envelope: 

  1. Your “Notification Card” printed with your My Number and an Application Form for Issuance of an” Individual Number Card.
  2. A return-mail envelope
  3. An explanatory pamphlet

Please take good care of your Notification Card in order not to lose it.

Those who wish to be issued with theirs are asked to complete the “Application Form for the Issuance of an Individual Number Card.”  Please consult with school and other family members first, and then apply for your My Number Card if it is felt to be necessary.

What to do if the details listed on your Notification Card have changed

Please go to the General Information Center Section at the Toshima City Office, or the inquiries counter at either the Tobu or Seibu Residents Offices in order to request the city office to print your most up-to-date details on the reverse side of your Notification Card.

Either you yourself or a member of your household visit your neighborhood inquiries counter with the following items.

  • Notification Card: (Please note that a Notification Card cannot be used as the proof of an individual’s identity).
  • Formal identification (a Residence Card, etc.)

What is a My Number Card?

A My Number Card may be used as a formal means of identification with photograph. Applying to be issued with a card is voluntary. (An application is free-of-charge at the first time).

Either extend your period of stay in Japan or change your residential address, you need to visit the Toshima City Office so as to complete certain administrative procedures.

Applying for a My Number Card

  • Applying by mail

To the “Application Form for Issuance of an Individual Number Card” that was included when your Notification Card was originally delivered, you are asked to affix a portrait photograph of yourself, and then to post the application form off in the return envelope provided. If you change your address or the expiry date of your qualification to remain in Japan after you received a Notification Card, you can not apply for a My Number Card using the “Application Form for the Issuance of an Individual Number Card” that was originally delivered with your Notification Card. Rather, you need to visit the Toshima City Office to receive an “Application Form for the Issuance of an Individual Number Card” that lists your most up-to-date details and apply for the card with it.

  • Applying online

It is also possible to apply for a My Number Card online by using portrait photographs taken with a digital camera or smartphone. This can be done by accessing the “Issuance of an Individual Number Card” website, entering the required information, uploading a portrait photograph, and then submitting the online form. You need to have the ID listed on the “Application Form for the Issuance of an Individual Number Card” for certain item to make online application. You are asked to make sure that you enter the ID of the “Application Form for the Issuance of an Individual Number Card” that lists your most up-to-date details. If the “Application Form for the Issuance of an Individual Number Card” does not list your most up-to-date details, you need to visit the Toshima City Office where you will receive an “Application Form for the Issuance of an Individual Number Card” that lists your most up-to-date details. That application form should then be used. If an incorrect ID is entered, a My Number Card shall not be issued.

Receiving an Individual Number (My Number) Card

Once preparations for the issuing of a My Number Card have been completed, you will be notified by the Toshima City Office. Once this notification has been received, please make an appointment by telephone or via the internet to visit the Toshima City Office. Please visit by yourself to the inquiries counter with the necessary items listed in the notification. .

*When receiving your My Number Card, you are asked to set your security PIN.

The validity of My Number Cards held by foreign residents is limited to the date of expiration of their period of stay. However, this stipulation does not apply to persons who are either permanent residents or special permanent residents.

As for persons who have filed an application for an extension of their period of stay in Japan with the Immigration Bureau, they need to visit their local municipality and complete the necessary procedures to extend the validity of their My Number Card prior to the expiration of their current period of stay in Japan.
